DISCLAIMER

I have read and understood the risk assessments provided by Brainwave and the participating organisations for this event. A copy of the risk assessment can be accessed by downloading the file below.

I declare that I am fit to take part in the event and agree to inform Brainwave of any new medical conditions that may affect my ability to take part in the event.

By booking a place at the 2019 Cheshire Clay Shoot, I hereby RELEASE, WAIVE, DISCHARGE, The Brainwave Centre Ltd, its employees, officers, sponsors, event organisers and volunteers from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my person, or to any property belonging, while taking part in the lead up to the event, or, on the day of the 2019 Cheshire Clay Shoot event to be held at Colshaw Hall on Thursday 19 September 2019.

Firearms

Under section 21 of the Firearms Act 1968 I declare that I am not a person prohibited by law from being in possession of a firearm by any of the following terms:

  • A person who has been sentenced to preventive detention, or to corrective training for a term of three years or more, shall not at any time have a firearm or ammunition in his possession.
  • A person who has been sentenced to corrective training for less than three years or to imprisonment for a term of three months or more, but less than three years, shall not at any time before the expiration of the period of five years from the date of his release, have a firearm or ammunition in his possession.
  • A person who is subject to a recognisance to keep the peace or to be of good behaviour, a condition of which is that he shall not possess, use or carry a firearm; or subject to a probation order containing a requirement that he shall not possess, use or carry a firearm.

It is an offence for anyone to contravene any of the aforementioned provisions of the Firearms Act 1968.

I also declare that I have no medical condition or nervous disorder likely to cause a danger to myself or others whilst I am in possession of a firearm.

Publicity

By attending this event I give consent for Brainwave to publish photographs, video or audio recordings of, or including me, in Brainwave publications or websites or in external outlets including newspapers or social media sites.  These images can be used at any point in the future without limitation and without charge.
